Registration Information

You will need to have the following documents ready to upload when you register your child:

  • Proof of residency: You will need to provide 2 forms of proof of residency (utility bill,         lease, contract, etc.) with your name and address                                                                         * If you have no bills in your name, you will need to get an Affidavit from the front office. This form will need to be filled out and notarized in 2 places. 2 forms of proof of residency will need to be supplied along with this Affidavit. In addition, a representative from the middle school will need to meet with the parent/guardian and with the person listed on the proof of residency paperwork.​
 
  • Birth certificate                                                                                                                                                                                 
  • Custody Papers, if applicable                                                                                                                               
  • TN Immunization Form                                                                                                                    
Withdrawal Information
In order to withdraw your child from school, you will need to come to the school and fill out a withdrawal form. Your child will need to bring all books and materials that belong to the school on their last day so they can turn those in. We will give you a copy of this form to take with you to register your child for their new school. 

* If you are withdrawing your child to an umbrella/church-related home school, we will need proof of registration with the home school before we can officially withdraw your child from school. If your child will be independently home schooled, you will need to bring a copy of your high school diploma or GED and fill out an Intent to Home School form.
